Based on direct observation, the facility failed to maintain the sprinkler system as required by NFPA 101 4.6.1.2, 9.7.5, 19.3.5, and NFPA 25, 5.2.1.1.1.

Findings include:

1. On 02/08/10, the day of survey, during the exterior tour conducted from 9:45 to 10:10 a.m., direct observation, accompanied by facility staff, revealed a sprinkler head " listed " escutcheon missing on 1, (one), of 18 sprinkler heads on the loading dock.

2. On 02/08/10, during the interior tour conducted from 10:20 a.m., to 2:05 p.m., direct observation, accompanied by facility staff, revealed 3 of 4 pendant sprinkler heads encrusted with foreign material in the Nursing Station located across from Room 224.

3. On 02/08/10, during the interior tour conducted from 10:20 a.m., to 2:05 p.m., direct observation, accompanied by facility staff, revealed 1 of 5 pendant sprinkler heads encrusted with foreign material in the 2nd floor Staff Lounge.

4. On 02/08/10, during the interior tour conducted from 10:20 a.m., to 2:05 p.m., direct observation, accompanied by facility staff, revealed 1 of 2 pendant sprinkler heads encrusted with foreign material in the kitchen dish washing room.

5. On 02/08/10, during the interior tour conducted from 10:20 a.m., to 2:05 p.m., direct observation, accompanied by facility staff, revealed 1 of 1 pendant sprinkler heads encrusted with foreign material in the Administrative Offices equipment room.
