Based on record review and interview, the facility failed to ensure 1 (#15) of 1 Medicare patients out of a total sample of 19 patients received the Important Message from Medicare resulting in the potential for all Medicare patients and/or patient representatives at this facility to not have the necessary information to exercise their rights. Findings include:
During medical record review on 12/20/2016 at 1510, it was revealed that Patient #15's medical record failed to have the required Important Message from Medicare (IMM) provided 48 hours prior to discharge. During an interview with Staff D on 12/20/2016 at 1538, it was confirmed that the second IMM was not in the medical record.
