Based on discussions on the morning of 10/17/2011, with the Administrator and Chief Operating Officer, the facility failed to contact the state health department prior to remodeling construction and replacement of generator. Additionally no " Final Architectural Inspection " forms were issued on the following construction: 1) the modifications of walls moved and 2) walls added in dialysis and 3) moving med gas lines and 4) in the emergency suite, the med room was added where a patient room was removed and 5) replacement of generator. The facility must request inspections on all items mentioned above and any other construction work conducted at the facility that did not have the State Health Department ' s Architectural Review Group approval.
