On 3/29/2016 between 9:30 AM and 2:30 PM, during the inspection a penetration was observed around cables above the ceiling tiles in the 2 hour fire separation between the CT Department and the Hospital.

FINDINGS INCLUDE:

During the Facility Inspection on March 29, 2016, between the hours of 09:30 AM and
2:30 PM, the following was observed:

a.) 2 extension cords were observed being used as fixed wiring in the Medical Records Office.
b.) 2 extension cords were observed being used as fixed wiring in the Boiler Room.
c.) a electrical junction box above the lay in ceiling at the the south/elevator smoke barrier was observed without a cover.

These defieicent practices were also observed by the Maintenance Supervisor.
