Based on a review of facility documents and staff interviews (EMP), it was determined that the facility failed to establish a direct delineation of accountability to the President of Excela Health Westmoreland Hospital for the Director of Rehabilitation and Geriatric Services of Excela Health Westmoreland Hospital's inpatient rehabilitation unit (Barclay Acute Rehabilitation Services).
Findings include:
1) A review of Excela Health Westmoreland Hospital's Organization Chart revealed that the position of Director of Rehabilitation and Geriatric Services of Excela Health Westmoreland Hospital's inpatient rehabilitation unit (Barclay Acute Rehabilitation Services) is not listed.
2) A review of Excela Health Latrobe Hospital's Organizational Chart revealed a direct reporting line from the Director of Rehabilitation and Geriatric Services of Excela Health Westmoreland Hospital's inpatient rehabilitation unit (Barclay Acute Rehabilitation Services) to the President of Excela Health Latrobe Hospital.
3) A review of the Excela Health (Employee Specific) Job Description 1816 - Director of Rehabilitation/Geriatric Services revealed that this position is under the Latrobe Hospital Administration Department and reports to the Latrobe Hospital President. The Job Summary revealed, "This role exits to assure the growth in our Barclay Acute Rehab services, located within the Neuroscience Center; assures access to our services in new and existing markets, and assures compliance with key quality and regulatory metrics. This role also serves a leadership and liaison function for the planning and coordination of care for senior services. ... II. Specialty Essentials Functions: 1. Clinical Excellence: Ensures that all patients are provided with quality care within the guidelines set forth by the Department of Health. A. Ensures quality patient care. Compliance with regulations, laws and insurance issues. ... c. Monitors and ensures that patients continue supportive services with Excela Health within parameters of freedom of choice. ... . "
4) Interview with EMP1 on April 14, 2010, at approximately 1:15 PM, confirmed the findings related to the organizational charts and stated that there is no contract between Excela Health Westmoreland Hospital and Excela Health Latrobe Hospital related to the Director of Rehabilitation/Geriatrics Services.
5) Interview with EMP2 on April 14, 2010, confirmed the findings related to the Director of Rehabilitation/Geriatric Services job description.
