Based on observation and interview, the facility failed to ensure safe hot water temperature control on showers in one facility.

Findings include:

1. On 11/1/2018, at approximately 3:05 pm., with N1 (Director of Risk) in patient room 508 A, the following was observed. The temperature of the water in the shower became too hot to touch.

2. Interview 11/1/2018, at approximately 3:05 pm., with N1 in patient room 508 A, confirmed the temperature of the water in the shower became too hot to touch

3. Interview on 11/1/2018, at approximately 3:10 pm., with patient 11, confirmed the water in the shower was too hot.

4. Interview on 11/1/2018, at approximately 3:20 pm., with N2 (Director of Plant Operations) confirmed 1:20 degrees F is the highest temperature that should come out of faucet.

5. Interview on 11/1/2018, at approximately 3:49 pm., with N2 confirmed he had adjusted temperature down before meeting with surveyor.

6. On 11/1/2018, at approximately 4:00 pm., in room 508 A, with N1 and N2 the following was observed. The water temperature coming out of the shower was 122.7 degrees F. Showers were push button and the temperature was not adjustable by patient.
