Based on observation and interview, the provider failed to maintain two randomly observed hallways (staff lounge hallway and doctors lounge hallway) clear and unobstructed. Multiple pieces of equipment including various types of beds and other kinds of equipment were stored in both hallways. Findings include:

1. Random observation on 8/10/15 from 2:30 p.m. to 5:00 p.m. of the staff lounge hallway and the doctors lounge hallway revealed various different types of medical equipment including beds were stored in the hallways.

Random observation on 8/11/15 at 7:00 a.m. of the staff lounge hallway and doctors lounge hallway revealed the above equipment and beds were still stored in the hallways.

Interview with the director of environmental services on 8/11/15 at 7:30 a.m. revealed:
*He was aware there was an issue with storing medical equipment in those hallways.
*He agreed the hallways were not maintained clear and unobstructed.
*The issue was also identified during the provider's accreditation survey in July 2014.
*He was working on a plan to develop more storage.
