A Life Safety Code (LSC) complaint survey (ASPEN #08PJ21; KS00147057) conducted by the Kansas State Fire Marshal's Office, Prevention Division on October 23, 2019 resulted in non-compliance with one Condition of Participation (CoP): C-0220, 42 CFR 485.623 , requirements for Physical Plant and Environment. The hospital failed to maintain the medical gas and vacuum piped system in accordance with National Fire Protection Authority (NFPA) 99, 2012 edition.
Findings Include:
The Philips County Health System is a two story building with a basement of type II protected noncombustible construction, and was built in 1955. The facility is partially sprinklered and all existing areas and any new construction will eventually be fully sprinklered. There is a manual fire alarm system with supervised smoke detection throughout. Emergency lighting is powered by an on-site generator and exit lights contain additional battery-backup.
The facility has a capacity of 25 with a census of 9 at the time of the survey.
In the manifold room, the interior location is not properly ventilated per NFPA 99, 2012 edition sections 9.3.7.5.1 through 9.3.7.5. The oxygen inlet/outlet in room 121, the oxygen inlet/outlet for the medical air recovery room 2, and the oxygen inlet/outlet for the medical air in the labor and delivery areas were leaking and not compliant with NFPA 99 2012 edition section A5.1.5.15. And the vacuum pressures in the minor OR procedure area do not comply with NFPA 99, 2012 edition sections 5.1.12.3.10.2 through 5.1.12.3.10.5.
On October 25, 2019, while reviewing the routine state licensure inspection report, the Office of the State Fire Marshal obtained a copy of the medical gas inspection report dated July 30, 2019.
During an interview with Maintenance Staff A, he was advised that the findings from the annual medical gas report have not yet been completed. Phillips County Health Systems reached out to the vendor on Wednesday October 23, 2019 to request an expedited quote on the deficiency repairs.
