Based on interview and facility document review, it was determined the facility failed to ensure one (1) of seven (7) staff members had up to date and documented competencies related to patient care.

The surveyor reviewed seven (7) staff member personnel records the afternoon of January 9, 2024 with Staff Members #2, #9, #10, #11, #12, #13 and #14.
During the review of Staff Member #8's competencies, it was determined that Staff Member #8 did not have an up to date and completed "Competency Based Orientation Tool Certified Nursing Assistant Level 1". Staff Member #8 lacked signatures from their leadership that documented competencies in bladder scanning. Staff Member #8's date of hire was July 11, 2022.
During an interview with Staff Member #9, the afternoon of January 9, 2025, when the surveyor asked what the expected timeframe is for documented competencies on the "Competency Based Orientation Tool Certified Nursing Assistant Level 1," they indicated that it is usually completed within four (4) weeks after hire.
