Surveyor observed between 11:00 and 11:30 am on 2/11/15 that in the basement kitchen, only one exit coudl be observed from the middle of the preparation area. The second exit was around a corner and the signage was not observable until almost reaching the door.

Facility failed to maintain electrical receptacle testing records in accordanc with the 2000 edition of the NFPA 101 and by reference NFPA 99, The Standard for Health Care. Findings included the following:

Surveyor observed between 10:00 and 10:30 am on 2/11/15 that no records of patient care area electrical receptacle testing was available. Receptacles shall be tested in Patient Care areas as follows(NFPA 99, 1999: 3-3.3.3):

Physical integrity of each receptacle (visual)
Continuity of the grounding of each receptacle (test)
Correct polarity of hot and neutral connections in each receptacle (test)
The retention force of the grounding blade shall be not less than 4 oz. (test)

Testing intervals:
General care areas tested annually
Critical care areas tested semi-annually
wet location tested annually.
