Based on medical record review and interview the facility did not ensure an appropriate discharge plan was in place for 1 of 11 patients (Patient #1).

Findings include:

Interview with Staff #7 Social Worker on 2/5/15 revealed a discharge planning meeting was held regarding the patient on 12/8/14. During the meeting it was noted that the patient's wife would be seeking a restraining order against her husband.

Review of the facility ' s discharge planning note dated 12/11/14 at 1:21PM revealed the social worker received an email from the patient ' s wife stating she would like to take the patient home today. The social worker documented that she had a discussion with the wife and made sure she understood everything that will be set in place.

Review of discharge notes dated 12/11/14 at 4:00PM revealed the patient was discharged to home in the care of his wife.

There is no evidence to indicate the facility definitively determined the status of the restraining order prior to the patient's discharge.
