Based on observation, it was determined that the facility failed to separate storage spaces with smoke resisting partitions.

Findings include:

1. On 1/24/12 at 10:55 AM, it was discovered that the wall of the chemotherapy storage room had a vent to the corridor.

2. On 1/24/12 at 11:10 AM, it was discovered that the corridor wall of the bed repair shop had three penetrations.

3. These findings were confirmed by Staff #21.

Based on observation, it was determined that the facility failed to provide not less then two exits in each fire section.

Findings include:

1. On 1/25/12 at 10:50 AM, at the ground floor west corridor, only one exitway sign was visible.

2. This finding was confirmed by Staff #21.

Based on observation, it was determined that, the facility failed to ensure Alcohol Based Hand Rub dispensers (ABHR) are not installed above ignition sources.

Findings include:

1. On 1/25/12 at 11:30 AM, in the special care nursery, on the south wall, it was discovered that an ABHR was installed above an electrical outlet.

2. This finding was confirmed by Staff #21.
