Based on Fire Drill Documentation review and interview, it was determined the facility failed to conduct a fire drills on the second shift (night shift) on five of eight quarters for the years 2009 and 2010. Failure to conduct fire drills had the potential to affect patient safety from fire because proper fire response training was not tested and evaluated. The failed practice had the potential to affect all staff, visitors, and patients admitted to the facility with an average daily census of 28 patients. The findings follow:
A. Review of the Fire Drill documentation provided on 01/12/11 at 1300 revealed fire drills were not conducted on the second shift for the third and fourth quarters of 2009 and the second shift in the first, third, and fourth quarters of 2010. The facility staffing consisted of two 12-hour shifts each day of the week.
B. In an interview on 01/12/11 at 1315, the Director of Plant Operations verified fire drills were not conducted for the shifts with no documentation and stated there was no further fire drill documentation available for review.
