Based on observations, the facility failed to meet the required initiation the fire alarm panel as required by NFPA 101 section 19.3.4.4 and section 9.7.5, NFPA 25. This standard deficiency affected the entire facility on the day of survey.

Findings include:

On 11/6/18 at 12:20 PM, observation and testing revealed the sprinkler system inspector test valve initiated and activated the fire alarm system in 142 seconds. The initiation of the fire alarm system shall be activated in 90 seconds or less.

The finding was acknowledged by the Administrator and Maintenance Supervisor verified this observation during the exit interview on 11/6/18.
