Based on observation and testing the horizontal exit doors going into the "new: hospital area would not properly close and latch. This deficient practice could affect all building occupants in the event of a fire.

Findings include:

During the facility tour on 8-16-12 at 9:00 AM, the double cross corridor horizontal exit doors would not properly close and latch as required by LSC(00) Section 19.2.2.2.6. The doors were released from the hold open magnetic device and would not close as they were rubbing on the top edge.

This deficient practice was confirmed by maintenance worker (DP) at the time of exit.

Minnesota State Fire Code (07) Edition Section 312.1. Requires fuel tanks be protected with guard posts from vehicle impact. This deficient practice could affect all patients, visitors, and staff in the event of a collision.

Findings include:

During the facility tour on 8-16-12 , it was observed that the fuel tank of the generator is not protected from potential vehicle impact, as required by MN State Fire Code section 312.1. The generator is located at the building edge of the east parking lot by the Emergency Department.


This deficient practice was confirmed by maintenance worker (DP) at the time of exit.

Findings include:

During the facility tour on 8-18-12 at 9:45 AM, it was observed that an extension cord was being used to supply power to equipment in the "new" lab. The use of extension cords as a substitute for permanent wiring is not allowed per NFPA 70.

This deficient practice was confirmed by maintenance worker (DP) at the time of exit.
