Based on record review and interview, the facility failed to ensure each patient had a discharge summary completed by a medical doctor within 30 days following discharge.
Findings included:
No discharge summary or final progress note found for patient #2. Medical records were also unable to locate either documents.
The above was confirmed with staff members #2 and 3.
