Based upon observations made in the presence of the plant manager on 8/15/12, it was determined that the facility did not maintain corridor fire separation of the facility in accordance with NFPA 101: 19.2.2.2.6 and 7.2.1.8.2.

Findings include:

1. Resident room #212 had a 4 " wall opening behind the resident bed that would not resist the passage of smoke and fire.
2. The back kitchen door to the corridor by the elevator was blocked open by a door wedge.
3. Resident room #303 had a 4 " x 4 " wall opening that would not resist the passage of smoke and fire.
4. The 3rd floor shower room door had three ? " openings through the door that would not resist the passage of fire and smoke.

Based upon observations made in the presence of the plant manager on 8/15/12, it was determined that the facility did not maintain the exits to be readily accessible at all times in accordance with NFPA 101: 19. 2.2.2.4. and 7.2.1.6.1

Resident room #310 and #308 had hook locks on the outside frame of the restroom door that may prevent egress from the room.

Based upon observations made in the presence of the plant manager on 8/15/12, it was determined that the facility did not maintain the fire sprinkler system in accordance with NFPA 101: 19.7.6 and 4.6.12, NFPA 13: 3-2.6.3., 5.1.2. NFPA 25: 9-2.3.

Findings include:

The laundry room closet had boxes stored in the closet with in 18 " of the sprinkler head that would obstruct the fire sprinkler spay pattern.
