Based on observation and interview, it was determined 5 of 36 medical gas cylinders were not secured to prevent them from falling. The failed practice had the potential to affect staff members who work within in the Med Gas Room due to the potential of injury presented by a cylinder falling over. The failed practice had the potential to affect all staff working in the Med Gas Room due to the potential for the cylinders to become a projectile if the cylinder heads were knocked off in the cylinders fell over. Findings follow:

A. During a tour on 006/08/2016 at 0900, 5 medical gas cylinders were observed unsecured by a chain or other means to prevent them from falling over.
B. The Maintenance Director verified the cylinders were unsecured at the time of observation.

Based on Generator Log review and interview, it was determined the facility failed to perform 12 of 12 (June 2015-May 2016) monthly generator tests under load as required. Failure to perform the tests, under load conditions, had the potential to affect all patients, staff, and visitors because the facility could not assure that the generator would supply emergency power in the event of a power outage. Findings follow:

A. During review of the Generator Log on 06/08/2016 at 1030, it was discovered the facility failed to perform the required 30 minute generator test, under load conditions, for 12 of 12 months from June 2015 to May 2016.
