Based on observations, the facility failed to properly protect hazardous areas.

Findings Include:

On February 24 at 1:00 p.m., the following hazardous areas located on the 3rd floor were lacking automatic door closers.
1) Room 322
2) Room 326
3) JPB lab
4) Report room

The freezer equipment room on the 1st floor also lacked an automatic door closer. All of the areas contained combustible or flammable storage and were greater than 50 Sq. Feet.

Based on observations and testing, the facility failed to provide door locks that are accessible from the egress side of the door and the locks at the required height.

Findings Include:

On February 24, 2015 between 2:00 p.m. and 2:50 p.m., the following locations in the behavioral unit had locks that were not accessible from the egress side:

1) Utility Room
2) Wheelchair Storage Room
3) Clean utility
4) Soiled utility
5) Environmental services
6) Storage closet

Additionally, the locks on the exit doors were greater than 48 " .

7.2.1.5 Locks, Latches, and Alarm Devices.

Doors shall be arranged to be opened readily from the egress side whenever the building is occupied. Locks, if provided, shall not require the use of a key, a tool, or special knowledge or effort for operation from the egress side.

7.2.1.5.4*
A latch or other fastening device on a door shall be provided with a releasing device having an obvious method of operation and that is readily operated under all lighting conditions. The releasing mechanism for any latch shall be located not less than 34 in. (86 cm), and not more than 48 in. (122 cm), above the finished floor. Doors shall be operable with not more than one releasing operation.

Based on observations, the facility failed to provide a complete manual fire alarm system as directed by NFPA 101 9.6, and NFPA 72 4-4.4.2.3.

Findings Include:

On February 24, 2015 at 2:30 p.m., the signaling devices in the 4th floor crossover corridor were not synchronized.

In corridors where there are more than two visible notification appliances in any field of view, they shall be spaced a minimum of 55 ft (16.76 m) from each other or they shall flash in synchronization.
