Based on observation and document review, the provider failed to meet the minimum construction standards of the 2000 Life Safety Code (LSC) for health care occupancy. Findings include:
1. Observation at 10:00 a.m. revealed the building was a two story, slab on grade, protected ordinary Type III (211) structure with a partial basement. The building was not equipped with a complete automatic sprinkler system. The patient bed wing built in 1959 was a single story, slab on grade, protected non-combustible, Type II (111) structure. The 1959 addition was not separated from the original building with a two hour fire resistive separation. Review of previous survey data confirmed that condition had existed since the building construction.

Based on observation and document review, the provider failed to ensure two conforming exits were provided from each floor of the building. Findings include:
1. Observation at 9:00 a.m. revealed the basement level of the building contained no conforming exits directly to the exterior of the building. One exit route was an interior stair that discharged into the first floor corridor. The second access to the exterior was through the boiler room to an open exterior stairwell. Document review of previous survey data indicated those conditions had existed since the original construction.
2. Observation at 9:30 a.m. revealed the second floor level of the building contained no conforming exits directly to the exterior of the building. One exit route was an interior stair that discharged into the first floor corridor. The other route was across the building roof to a fire escape stair. Document review of previous survey data indicated those exits were part of the original construction.

Based on observation and measurement, the provider failed to maintain conforming exit ramps, stairs, and guards. Findings include:
1. Observation at 11:45 a.m. revealed the boiler room exterior stairwell and the north fire escape stair from the roof were not provided with 42 inch high guardrails. The openings in the existing railing exceeded the six inch spacing for existing conditions. Measurements identified the guardrail height was 36 inches and an opening spacing of several feet.
2. Observation at 1:00 p.m. revealed the west exit ramp from the patient wing exceeded the maximum allowable slope of 1:10 for existing ramps. The measurement indicated the actual slope of the ramp was 1:6.
