Based on observation, record review and interview the facility failed to document the emergency egress lighting monthly. This deficient practice affects all smoke compartments and all occupants of the facility. This facility has a capacity of 25 and a census of 11 patients.
Findings include:
Observation, record review and interview of the facility's maintenance records on 4/27/16 at 12:11 p.m. revealed that there was no documentation of the date regarding the testing of the 30 second emergency battery lighting system. Maintenance Staff verified this observation during the survey process.

Based on observation, staff interview and record review, the facility failed to maintain the emergency generator power supply as required. The deficient practice would affect all smoke compartments of building and all of the patients and staff. The facility has 25 certified beds and at the time of the survey the facility census was 11.

Observation, documentation review and staff interview on 4/27/16 at 12:03 p.m. with Maintenance Staff A revealed that proper documentation of the generator weekly inspections and run times had not been maintained. Documentation consisted of a log sheet that did show the start and end times of the testing of the generator. Maintenance Staff verified this observation during the survey process.

Based on observation and staff interview, it was determined the facility failed to maintain the buildings electrical wiring system in accordance with National Fire Protection Association (NFPA) Standard 70, National Electrical Code, 1999 edition, placing the Staff and Patients of the facility at risk in the event of a fire. The facility had a capacity of 25 and a census of 11 at the time of the survey.

Observation and staff interview on 4/27/16 at 1:30 p.m. revealed the facility failed to provide a Ground Fault Circuit Interrupter electrical outlet under the drinking fountain in the NW Surgery corridor. Maintenance Staff verified this observation during the survey process.
