Based on observation on January 23, 2024, at approximately 11:30 AM the following deficiencies were noted:
The facility inspection of the rated walls in the boiler room was non-compliant the specific items include:
After fire/smoke event in the Boiler room, there was evidence of smoke had entered the stairwell
The facility rated wall behind the right most boiler has an unsealed penetration where communications yellow wire intersects with the wall. This condition does not allow the enclosure to remain smoke tight to the adjacent stairwell and allowed smoke to enter the stairwell.
