Based on observation on April 27, 2011 with the director of services and the manager of physical plant poor high dusting housekeeping was discovered as evidenced by noticeable accumulation of dust as found on 70% of all examples tested throughout all patient care areas. Examples include dust; dirt; and lints can clearly be observed on high surfaces such as wall cabinets and staff lockers serving the rural clinic area and on wall cabinets in toilet room serving the fluoroscopy room west. Also, dirt/lint was seen on the ground under the control counter in the control room serving fluoroscopy west room also in the lab area next to the under counter refrigerator in the blood draw area.

Based upon on-site observation and document review by Life Safety Code (LSC) surveyors, the facility does not comply with the applicable provisions of the 2000 Edition of the Life Safety Code.
