Based on medical record review and staff interview, the facility failed to ensure that a comprehensive assessment of the residents' needs was conducted for Patient #1 and #2, two (2) of two (2) records reviewed.
Findings Include:
Record review for Patient #1 and #2 revealed that the physician ordered wound care for both patients who were post-operative hip surgeries. Neither of the patient's Interdisciplinary Care Plan addressed Skin Impairment.
During the Exit Conference on 8/27/14 at 10:15 a.m., the Swing Bed Coordinator and other staff members were present. The findings that the patient's Interdisciplinary Plan of Care did not include skin impairment was presented. No further information was provided.
