Based on a Life Safety Code survey conducted on February 1 and 2, 2016 it was determined that the Condition of Participation for Physical Environment was not met based on the following deficient practices noted in that survey ;

K-017 - Due to penetrations through the walls near the Decon Room;

K029 -Due to the lack of a self closing device on the door in the Medical Records Storage area;

K038 - Due the impediments of exit access in three areas;

K056 - Due to the lack of sprinkler heads or the types of sprinkler heads in two different areas;

K062- Due to the failure to perform required standpipe system testing within the required time frames;

K069 - Due to the failure to meet the building fire alarm interconnection requirement for the kitchen hood;

K070- Due to the use of electric space heater on a damaged electrical cord;

K072 - Due to the failure to maintain egress corridors unobstructed ;

K076 - Due to the various violations in the storage of medical gases;

K075 - Due to the failure large trash containers in areas that comply with Hazardous area requirements ; and

K0147 - Due to the failure to use of 15 amp extension cords or power strips being used for small appliances and space heaters
