Based on observation during the survey walk through, not all egress paths lead to an exit. This deficient practice could require a person to traverse a longer route in order to reach an exit. This condition may compromise the prompt care and movement of patients, visitors and staff during a fire/smoke emergency.
The finding is:
On 09/07/2017 at 11:05am while accompanied by the DOF, the main North/South Corridor located between Cardiac Rehab and Surgery does not comply with 19.2.5.1 providing two designated paths of egress.

Based upon observation, Exit signs are not provided to provide clear identification of exit access. Failure to identify available means of egress can result in occupant confusion or inability to reach an exit during a fire/smoke event.
The finding is:
On 09/07/2017 at 9:45am while in the company of the DOF exit signage was observed to be covered by soffits approximately 24 inches deep which does not comply with 7.10.1.2.1
Location observed - Outpatient Clinic T-intersection corridor adjacent to nurse station.

Based upon direct observation, sprinklered hazardous areas are not separated by smoke resisting construction with self-closing doors. Failure to separate hazardous areas can expose occupants to fire conditions before evacuation can occur.
The finding is:
On 09/07/2017 at 10:45am while in the company of the DOF it was observed that an access panel does not comply with 19.3.2.1, 19.3.2.1.3, 8.7.1 & 8.4.
Location observed: Kitchen drygoods storage wall adjacent to dishwashing three compartment sink, contains an access panel. The drygoods storage wall is 1-hour rated and the panel does not comply with 19.3.2.1.3 as self closing nor 19.3.2.1 for maintaining an existing 1-hour fire barrier.

Based on observation and interview, the building's fire alarm system is not installed in accordance with Code requirements. Failure to properly install components of a fire alarm system may compromise the operation of the system. The system may fail to provide an effective warning if there is a fire/smoke event.
The finding is:
On 09/07/2017 at 11:40am while accompanied by the DOF it was discussed that there is no fire alarm communication between the M.R.I. unit located in a trailor ( permanently mounted adjacent to the ambulance drive through) and the hospital. Currently this condition does not comply with NFPA 72-2010 24.4.1.

Based on observation, not all portions of the building fire alarm system are installed to notify all occupants. This condition may prevent emergency egress from the location of fire origin to an adjacent compartment. This condition may affect patients, staff and visitors throughout the facility.
The finding is:
On 9/07/2017, 10:15am while accompanied by the DOF a staff sleeping (on-call) room was observed to not comply with NFPA 101 2012 19.3.4.1, 9.6.2.10.1.1 and NFPA 72, 2010 ed 18.5.4.6.1 for the installation of a combination smoke alarm and visible notification (strobe) within the sleeping area.
Location observed: West end of building, Staff on-call room located adjacent to Trauma and Radiology.
