PATIENT RIGHTS: NOTICE OF RIGHTS

Based on interview and record review, the facility failed to inform the patient ' s representative in advance of the furnishing and the discontinuing of patient care in 1 of 3 patient charts (#83). Findings include:

During record review of patient # 83 ' s chart on 6/23/10 & 6/24/10, revealed that the facility failed to inform the patient ' s representative of both the admission and discharge of the patient. The patient was admitted through the emergency room on 4/3/2010 from a skilled nursing facility. During admission on the nursing floor R.N. # A admitted the patient at 1627 and documented two contact numbers for a family member. The family member was not called and notified of the patient admission.

PATIENT RIGHTS: NOTICE OF GRIEVANCE DECISION

Based on interview and file review, the facility failed to provide written notice of its decision in the resolution of a grievance for 1 of 3 (#83) grievances reviewed. Findings include:

Interview with the Patient Relations Coordinator, on 06/24/10 at approximately 1130 revealed that there was a grievance that entered the system on 04/08/10. A letter dated 04/09/10 from the facility, acknowledged the receipt of the grievance. The patient representative called the facility again on 05/04/10 because they were told in the letter dated 04/08/10 that they would be contacted as soon as the review had been completed. A second letter dated 05/06/10 was sent to the patient representative stating that they would be contacted as soon as the facility has received verification from the physician on the patient ' s case. Again, there was no closure to the complaint that was made. According to the policy titled " Patient & Family Grievances and Complaints " the facility will " Follow up as necessary, including feedback to complainant within 30 days. " File review and contact with the physician revealed that completion of the investigation and the decision correspondence had not been completed as required when reviewed on 06/24/10.

DISCUSSION OF EVALUATION RESULTS

Based on record review and interview the facility failed to discuss the results of the discharge evaluation with the patient ' s representative, for 1 of 3 medical records reviewed (#83). Findings include:


On 06/23/10 at approximately 1430 during the review of patient #83's medical record, the social worker wrote a discharge note on 04/07/10 at 1004 stating that the patient "now ready for transfer back (SNF), updated patient info faxed to (SNF)" and that the "ambulance was set for 1200 noon " . There was no patient representative listed on the discharge instructions. There was no other documentation that supported that a discharge evaluation was discussed was discussed with the patient ' s representative.

During an interview on 06/24/10 at 1100 the Vice President of Patient Care Services and the Recipient Rights Advisor confirmed these findings.
