Based on a review of facility documentation and staff interviews, the facility failed to perform a fall risk assessment as required in facility policy in 1 of 10 patient charts reviewed.
FINDINGS WERE:
The Emergency Department record of Patient #1 included no documented evidence of a fall risk assessment as required by facility policy.
Facility policy #PC.TX2-001, entitled Fall Prevention Program, approved 2/2012, stated in part:
"PURPOSE: To identify patients at risk for falls and develop guidelines to reduce fall incidence.
POLICY: ...Outpatients: All outpatients will be evaluated for fall risk at the time of service provision, and should conditions change during the time of service..."
The above findings were confirmed in an interview with the Chief Nursing Officer and Emergency Department Director on the afternoon of 2/4/14 in the facility meeting room.
