Based on policy and procedure review, observations during tours and staff interviews, the facility staff failed to ensure nourishment refrigerator temperatures were monitored daily for 3 of 4 units toured.

The findings include:

Review conducted June 29, 2016 of the facility's policy, "Refrigerators, Freezers and Ice Machines, Care of" revised June 2016 revealed, "I. SCOPE / PURPOSE...II. POLICY All refrigerators, freezers, ice machines will be kept clean, contain the appropriate items and kept at the appropriate temperature to ensure the safe storage of food or medications. A. Patient Nourishment Refrigerators/Freezers 1...7. In patient care areas; the thermometer must be checked daily and the temperature the refrigerator/freezer documented..."

1. Observation during unit tours conducted June 28, 2016 at 1150 revealed Unit 1, an inpatient care area. Observation of the June 2016 patient nourishment refrigerator monitor log revealed 18 of 28 days no temperatures were monitored.

Interview conducted June 28, 2016 at 1150 with Nurse 1 revealed the patient nourishment refrigerator temperatures were not monitored daily.

2. Observation during unit tours conducted June 28, 2016 at 1410 revealed Unit 2, an inpatient care area. Observation of the June 2016 patient nourishment freezer monitor log revealed 10 of 28 days no temperatures were monitored.

Interview conducted June 28, 2016 at 1428 with Unit Manager 1 revealed the patient nourishment freezer temperatures were not monitored daily. Interview revealed any clinical staff member was allowed to monitor the temperature.

3. Observation during unit tours conducted June 28, 2016 at 1428 revealed Unit 3, an inpatient care area. Observation of the June 2016 patient nourishment refrigerator monitor log revealed 8 of 28 days no temperatures were monitored.

Interview conducted June 28, 2016 at 1428 with Unit Manager 2 revealed the patient nourishment refrigerator temperature were not monitored daily. Interview revealed the certified nursing assistant was responsible for temperature monitoring.
