Based on observation and document review, the provider failed to ensure two conforming exits were provided from each floor of the building. Findings include:
1. Observation at 11:00 a.m. on 12/16/14 revealed the basement level of the building contained no conforming exits directly to the exterior of the building. One exit route was an interior stair that discharged into the first floor corridor. The second access to the exterior was through the boiler room to an open exterior stairwell. Document review of previous survey data indicated those conditions had existed since the original construction.
2. Observation at 11:30 a.m. on 12/16/14 revealed the second floor level of the building contained no conforming exits directly to the exterior of the building. One exit route was an interior stair that discharged into the first floor corridor. The other route was across the building roof to a fire escape stair. Document review of previous survey data indicated those exits were part of the original construction.
The building meets the FSES. Please mark an "F" in the completion date column to indicate correction of the deficiencies identified in K000.
