PATIENT RIGHTS: NOTICE OF RIGHTS

Based on review of records and interview with staff, the facility failed to provide information regarding patient rights in advance of furnishing treatment for 1 of 1 patient whose medical record was reviewed.
Findings were:
The facility provided for review a copy of the patient rights information that is given to each patient along with a signature page that is part of the medical record. There was no documentation that the patient rights information was given to Patient #1. This was verified in an in-person interview with Staff #1 the morning of 10/19/2010.
MAINTENANCE OF PHYSICAL PLANT

Based on observation and interview with staff, the facility failed to ensure that the hospital environment was maintained in a safe manner for 1 of 1 patient whose record was reviewed, as the environment presented possible safety and infection control hazards.
Findings were:
Patient #1 was inpatient in two units of the hospital - the Psychiatric Intensive Care Unit (PICU) and the Adult Unit. A tour of the Adult Unit was conducted on 10/19/2010 in the company of the facility risk management director, Staff #1. The showers in rooms 235 and 236 had peeling, crumbling plaster below the shower heads where the water had damaged the areas.
The PICU was observed during the tour on 10/19/2010. The unit has a communal shower, and the floors, walls and ceilings were dirty. The grout between tiles was blackened, 2 sprinkler heads were uncovered, and all vents were rusted. Tiles were missing from the walls and a seat that was attached to the wall was rusty and separating from the wall. The bottom of the egg tub (used for bathing wheelchair patients) was encrusted with black material and a wet pair of underwear was noted behind the tub.
Rooms #1 and #2 in PICU had paint peeling around the drop-in shower unit. Additionally, there was a 7" x 7" patch of glue residue in the shower unit which has accumulated dirt. A mattress In Room #5 has a one-inch tear in the seam of the mattress. Staff #1 verified these findings during the tour of the units.
