Based on an interview and medical record (MR) review, hospital staff failed to assure the patient's plan of care addressed the patient's identified needs and was updated to reflect patient's specific medical conditions. This deficient practice effected 11 of 11 sampled inpatient records and had the potential to affect all patients at this facility.The findings include:A review of 11 of 11 inpatient medical records revealed there was no individualized care plans. The care plans did not contain individualized goals for the patient's diagnosis nor individualized interventions. The information listed on the care plan was the same for each diagnosis.Employee Identifier # 1. the Quality Coordinator, was interviewed on 1/14/14 at 9:20 AM, and verified the care plans were not individualized.

Based on observations during facility tour with hospital staff by the Fire Safety Compliance Officer and staff interviews, it was determined that the facility was not constructed, arranged and maintained to ensure patient safety. This had the potential to affect all patients served.
