A. Based on a review of Hospital policy and procedure, employee training records, and staff interview, it was determined that in 1 of 4 (Employee #1) employee training files reviewed, the Hospital failed to ensure all employees were current in their training requirements.

Findings include:

1. The Hospital's policy and procedure titled, "Safe Patient Handling" was reviewed. It indicated under IV. PROCEDURE: A. Training of Staff 2. All current employees whose duties include (or may include) lifting, transferring, repositioning, or otherwise assisting a patient will receive appropriate training on all lift/transfer devices. Competencies (including return demonstrations) will be completed and documented in the employee's file. 5. Annual re-education/competencies will be completed by the TMCs (Transfer Mobility Coach) and unit based Staff Education Coordinator."

2. The computer training files of Employee 1 (E-1) were reviewed. The training files indicated that E-1 completed the "Lifting and Transferring Patients (PA)" in 3/14/09. There was no documentation that E-1 received the required annual training since March 2009.

3. During an interview with the Director of Quality, conducted on 9/16/10 at 12:15 PM, the above finding was confirmed.
