Based upon observations and staff interviews on August 6, 2014 between approximately 1000 and 1430 hours Cascade Valley Hospital has failed to ensure there was no impediment to the closing of patient room doors.


The findings include:

1) Second floor, rooms 205, 206 and 207 - these rooms have overhead patient lifting equipment installed. This equipment requires the privacy curtains tobe installed parallel with the corridor wall. Doors to these rooms have automatic release mechanisms installed. Upon activation of the alarm system, the door will not be allowed to fully close if the privacy curtain is drawn.

The facility must ensure the doors are allowed to fully close without impediment.

Based upon observations and staff interviews on August 6, 2014 between approximately 1000 and 1430 hours Cascade Valley Hospital has failed to maintain the fire sprinkler system as required. This could result in the failure of the fire sprinkler system to operate properly in the event of a fire and allow the fire to increase in size and intensity which would endanger the residents, staff and/or visitors within the facility.
The findings include:
1) Room 253 sprinkler head is located too close to overhead light fixture.

Based upon observations and staff interviews on August 6, 2014 between approximately 1000 and 1430 hours Cascade Valley Hospital has failed to restrict the use of multi-plug outlets (power strips) to providing power to permitted electrical equipment. This could result in a fire from overheating of the plug strip due to the heavy power draw endangering the residents, staff and/or visitors within the facility.
The findings include:
1) Room 151B, cord from a multiplug adapter was seen passing through an office door - CORRECTED AT SURVEY
2) Accounts Payable office two multiplug adapters were daisy chained together - CORRECTED AT SURVEY
3) Patient Financial Services there was an unapproved extension cord being used to power three appliances - CORRECTED AT SURVEY
