Based on observation and interview, the facility failed to maintain clear access to exits in accordance with NFPA 101 (2012 edition) 19.2.1, 7.1.10.2.1. This deficient practice affected 6 of 10 patients and an undetermined number of staff and visitors in the facility.
Findings include:
1. On 02/19/20 at 9:58 am, observation revealed that the exit access from Patient Rooms 10, 11 and 12 to exits was obstructed by 8 ft step ladders, several cans of paint, yellow tape and orange cones.
2. On 02/19/20 at 10:26 am, observation revealed that two exit doors in the east exterior wall of the unit were obstructed by two chairs placed in front of exits.
The above deficient practice was confirmed by interview with Staff B on 02/19/20 at 2:00 pm.

Based on record review and interview, the facility failed to properly maintain the fire alarm system in accordance with NFPA 101 (2012 edition) 19.3.4.1, 9.6.1.3 and NFPA 72 (2010) 14.4.5 Table 14.4.5(6)(d), 14.4.2.2. This deficient practice affected all patients and an undetermined number of staff and visitors in the facility.
Findings include:
On 02/19/20 between 12:45 pm and 2 pm, review of the fire alarm maintenance and inspection records revealed that the semiannual load voltage test of the sealed-lead acid type fire alarm batteries was not performed.
The above deficient practice was confirmed by interview with Staff B on 02/19/20 at 2:00 pm.

Based on observation, record review and interview, the facility failed to maintain electrical receptacles in patient bed locations in accordance with NFPA 99 (2012 edition) 6.3.4.1.3. This deficient practice affected 6 of 10 patients, and an undetermined number of staff and visitors in the facility.
Findings include:
On 02/19/20 between 10:15 am and 11:45 am, observation and record review revealed that the electrical wall receptacles installed in the vicinity of patient room sinks were neither hospital grade receptacles nor was there evidence of the GFCI wall receptacles tested at intervals not exceeding 12 months. When interviewed on 2/19/20 at 1:45 pm, Staff B stated that GFCI electrical receptacles were installed near sinks in all patient rooms.
The above deficient practice was confirmed by interview with Staff B on 02/19/20 at 2:00 pm.
