Based on review of hospital " Restraints and Safety Alternatives. . ." policy, patient chart, and interviews it was determined that the facility failed to discontinue restraints at the earliest possible time and order correct classification (violent or nonviolent) of restraint.
Findings were:
* Physician placed order for non-violent restraints. Charted behavioral rational for restraint order "Patient still combative, throwing objects from drawers, verbally aggressive with staff."
*Patient placed in restraints at 00:35 4/10/21. Patient behavior charted "calm" at 01:35. Restraints remained in placed until 02:20.
*Facility's "Restraint and Safety Alternatives- Use of- Medical/Surgical Acute Care Facilities" state violent restraints are used when "violent or self-destructive behavior jeopardizes the immediate physical safety of the patient, staff, or others" and should be discontinued when behavior no longer poses a risk.
