Observation revealed on 10/12/22 that not all required fire rated walls were completely sealed to prevent the passage of fire and smoke. Please verify all rated walls and partitions are completely by flush, taped and floted patches at all holes have been installed ("Scab-On" patches are prohibited) and pipe or wire penetrations are sealed with proper caulk.

On 10/12/22, the Maintenance Director stated that all rated walls and partitions will be properly sealed, particularly at the Mechanical Room.

Based on observation on 10/12/22, the facility failed provide an operating exhaust system at all required spaces, including janitor closets and restrooms. Please verify all required fans are functioning correctly.
