Based on document reviews and interviews, the hospital failed to ensure identified staff had certification in the use of cardiopulmonary resuscitation (basic life support) for 2 of 10 employees reviewed (Physician #1 and a Nurse Practitioner).

Findings:

The "Workplace Safety and Security" policy, last revised on 2/15/19, indicated, "All staff identified in the Staff Safety Training Matrix (attachment D) will receive training and certification initially and then every two years in BLS [Basic Life Support] accredited by the American Heart Association." Medical Services and Clinicians were identified in the Staff Safety Training Matrix as requiring BLS.

1. Physician #1's training record indicated his BLS expired on 1/31/19. As of 9/17/19, there was no evidence this employee had been recertified in BLS.

2. A Nurse Practitioner's training record indicated her BLS expired on 3/31/19. As of 9/17/19, there was no evidence this employee had been recertified in BLS.

On 9/17/19 at approximately 3:00 PM, the above findings were confirmed by the Director of Privacy and Compliance.
