Based on record review and interview, the facility failed to follow their fire plan policy.

The findings include:

Record review of the fire plan and interview with Director of Environment, Health, Safety and Emergency Management on 5/17/17 at 1:50 PM revealed the dietary staff did not activate the fire alarm by pulling the manual fire alarm pull station. The fire plan states that any person hearing "code red" called aloud are supposed to activate the building fire alarm by using the nearest manual pull alarm station.

The Director of Environment, Health, Safety and Emergency Management and Fire Systems Mechanic was present and acknowledged the deficiency during the exit conference on 5/17/17.
