Based on observation, the hospital failed to ensure self closing doors successfully close and latch in 3 applicable areas of the hospital.

Per observation on 4/3/17, accompanied by the Maintenance Director, the following self-closing doors did not close and latch:
1. Sixth floor penthouse;
2. Fourth floor Rutland East fire door; and
3. Endoscopy fire door.

Based on observation, the hospital failed to ensure stairways are in accordance with requirements in all stair towers in the hospital.

Per observation on 4/3/17, accompanied by the Maintenance Director, all stair towers in the hospital do not have guards (railings) which prohibit a 4 inch sphere from passing through as required.

Based on observation, the hospital failed to ensure storage in one area of the hospital was in compliance with sprinkler requirements.

Per observation on 4/3/17, accompanied by the Maintenance Director, the Dialysis storage room has storage above the 18 inch sprinkler clearance plane.
