Based on observation, it was determined that the facility failed to ensure that storerooms are separate from other spaces and doors are self-closing.
Findings include:
1. On 9/17/12 at 11:05 AM, in the presence of Staff #24, there were two penetrations in the west wall in the basement Medical Records room.
2 On 9/17/12 at 11:15 AM, in the presence of Staff #24, the basement environmental storage room did not have a door closer on the door.
