PATIENT RIGHTS: RESTRAINT OR SECLUSION

Based on document reviews and interviews, the hospital failed to ensure providers completed annual restraint training for two (2) out of four (4) providers who ordered patient restraints in 2023 (for Patients 3R & 4R).

Findings:

1. Franklin Memorial Hospital Policy titled "Restraint Use- Comprehensive Policy," last revised 01/08/2022 reads in part:

" ...Authorized LIPs [Licensed Independent Practitioner] include all Emergency Room practitioners and all LIPs on the Hospitalist Service ..."
And
" ...All acute care nursing staff and LIP staff ordering restraints will review the restraint policy and complete education annually ..."

2. Patient 3R's medical record was reviewed and revealed that Provider #1 ordered Patient 3R be placed in violent restraints on 07/03/2023.

3. Patient 2R's medical record was reviewed and revealed that Provider #2 ordered Patient 4R be placed in non-violent restraints on 12/03/2023.

4. At 3:41 PM on 01/09/2024, Quality Improvement Specialist confirmed that the facility has no evidence that Provider #1 or Provider #2 completed annual restraint training for 2023.
