Based on interview and record review, the facility failed to provide evidence that treatment is ordered by the physician for 1 out of 11 sampled patients (SP) #1.
Findings include:
Review of the Patient Care Notes showed that on 10/18/17 at 2:28 am, that the Foley (urine catheter) catheter is clotted and leaks around, and new Foley (catheter) is replaced.
Review of medical records on 2/20/2018 showed no evidence of an order to insert Foley catheter on 10/18/17 was provided.
Interview with Staff C on 2/19/18 at 4:21pm, She indicated that Foley (catheter) insertion is similar to medication administration, needs an order.
