Based on observation and testing, the facility failed to provide the one (1) hour fire rated construction (with 45 minute fire-rated doors) or an approved automatic fire extinguishing system in accordance with 8.4.1 and/or 19.3.5.4. This condition affected one (1) of five (5) smoke compartments.

Findings include:

On July 24, 2012 at 10:30 a.m., the maintenance person and the the surveyor found the Central Storage Room had two walls that were not built to the deck and improperly sealed.
To prevent the propagation of smoke to other areas in the building.

The maintenance person and the Assistant Administrator were notified during the exit conference.

Based on observations the facility failed to provide the required readily accessible exit access in an existing facility in accordance with 7.1, 18.2.1, or 19.2.1.

Findings Include:

While inspecting the means of egress on July 24, 2012 at 11:00 a.m., the surveyor observed chair and equipment storage throughout the the facility.

This deficient practice has the potential of affecting four (4) of four (4) smoke compartments. The Assistant Administrator as well as the maintenance director was notified during the survey as well as the exit conference.
