The findings included:
1. Observation of the kitchen on 7/14/14 at 10:35 AM revealed,a wall penetration above the double doors. National Fire Protection Association (NFPA) 101,8.3.6.1
2. Observation of the emergency water storage room on 7/14/14 at 10:38 AM revealed, a wall penetration NFPA 101,8.3.6.1
3. Observation of the emergency department on 7/14/14 at 10:39 AM revealed, a wall penetration above the double doors. NFPA 101,8.3.6.1
4. Observation of the pharmacy at on 7/14/14 at 11:22 AM revealed, ceiling penetrations. National Fire Protection Association (NFPA) 101,8.3.6.1
5. Observation above ceiling on the corridor at the ultra sound room on 7/14/14 at 11:47 revealed conduit not sealed. National Fire Protection Association (NFPA) 101,8.3.6.1
6. Observation of the boiler room on 7/14/14 at 12:05 PM revealed, numerous ceiling penetrations. National Fire Protection Association (NFPA) 101,8.3.6.1
The findings were verified by the maintenance director and acknowledged by the administrator during the exit conference on 7/14/14.
