Based on review of hospital documents and interviews with hospital staff, the hospital does not assure contract staff are adequately trained to meet the needs of the patients. One of three contracted personnel (Staff E,F,G ) did not have departmental orientation, competency, and evaluation for the specialized areas where they worked.

Findings:

1. On the afternoon of 11/15/2010 surveyors were provided three contract personnel files. There was no documentation provided indicating the Staff G had orientation, trainining or competency evaluation completed by the hospital administration prior to working at the hospital. This finding was reviewed with administration at the time of the exit conference. No further documentation was provided.
