MAINTENANCE OF PHYSICAL PLANT

Based on observation, interview and record review the hospital environment was not maintained to ensure the well-being of the patients in that, flies were observed by the surveyor in the adult psychiatric unit during the survey on 09/15/15 and the floor needed to be cleaned.


Findings Included:

1.) During a physical tour of Mayhill Hospital on 09/15/15 at 2:45 PM flies were observed flying in the hallway on the Adult Psychiatric Unit. The unit also held a large trash container secured in a wooden frame with a large uncovered opening. The trash was overflowing and there was remains of food left in the trash bins.

2.) The floor needed to be mopped and cleaned in the adult unit. The floor was observed to have soap scum and dust residue.

The hospital personnel #1 escorted the surveyor on the tour on 09/15/15 at 2:45 PM and in observation and interview confirmed that the flies were on the Adult Psychiatric Unit.

In a face to face interview on 09/14/15 at 9:00AM with hospital personnel #4, it was confirmed that flies were a problem in the hospital. Hospital personnel #4 confirmed through interview that the hospital does not have direct housekeeping staff after hours. Personnel #4 reported the mental health techs are serving a dual role, helping with housekeeping duties.

The hospital Infection control plant services/housekeeping policies dated 04/24/15 reflect, "Routine and special cleaning will be performed as scheduled and supervised by Director of Plant Maintenance to provide clean environment for patients...The housekeeping staff will be responsible to see that the following daily cleaning tasks are performed; Patient Lounge Areas-empty trash...Trash can and waste baskets-line with can liners, disinfect as needed."
