Based on observation, review of facility documents, and staff interview, it was determined that the facility failed to maintain a sanitary environment on one (1) out of one (1) units used to quarantine COVID-19 positive patients.
Findings include:
1. A tour of Cedar Hall A was conducted on 12/10/20 beginning at 11:20 AM in the presence of Staff #5. The unit was identified by Staff #5 as a COVID-19 positive patient isolation unit. The unit was not currently occupied by patient's and, as per Staff #6, had been terminally cleaned on 12/4/20 when the last patient left the unit. The following was observed:
a. In Bedroom F, dark brown staining was visible beneath the toilet tank down to the floor and tape was stuck to the outside of the bedroom door.
b. In Bedroom D, tape and residual paper were stuck to the outside of the bedroom door. A "Falls Risk" sticker was also adhered to the door.
c. In Shower Room 118, what appeared to be moistened wash cloths and an opened shampoo bottle were visible in one of the showers.
d. In Bedroom G, tape and a torn piece of a "Falls Risk" sticker were visible on the outside of the door.
e. In Bedroom B, peeling stickers were visible on the bedroom door.
f. In Bedroom C, damage to the door left exposed wood visible in multiple areas. This was not a cleanable surface.
g. In the "Doffing Room" a personal protective gown and hair covering that appeared to have been used were hanging from a coat rack.
(i) During an interview at 11:46 AM, Staff #5 stated that the personal protective gown and hair covering should not be in the "Doffing Room" after a terminal cleaning.
2. During an interview conducted during tour of the unit, Staff #6 stated that the fronts and backs of doors are cleaned as part of the cleaning procedures. Staff #6 stated that he/she tries to clean the tape and stickers off the doors but that it is difficult to remove them.
3. A review of the facility document titled "Daily High Touch Cleaning Checklist for Isolation Rooms Housekeeping Department", revealed that "... Doors/Door knobs ..." and "... Shower rooms ..." had been sanitized in Ceder Hall A on 12/4/20.
