MAINTENANCE OF PHYSICAL PLANT

Based on observation and interview, it was determined the facility did not maintain a clean environment and maintain the environment in a good state of repair due to an accumulation of dust and damaged walls, doors, chairs and peeling paint in 16 of 21 rooms observed. The failed practice had the potential to affect all patients admitted to the facility. The findings follow:

A. On a tour of the facility on 10/05/10 at 0845 with the Director of Plant Operations, the following observations were made:
1) On the sixth floor, in Patient Room 617 an accumulation of dust was observed on the patient headwall light fixture. The toilet paper roller was missing in the patient toilet room. An accumulation of dust was observed on the exhaust grill in the Quiet Room. An accumulation of dust was observed on the patient headwall light fixture and toilet exhaust grill in Patient Room 606. The exhaust grill in the Janitors Closet on that floor was clogged with dust.
2) On the fifth floor, a section of the wall paint in Patient Room 519 was chipped and bubbling up. A portion of the wall in Patient Room 518 was peeling and the edge of the door to the room was damaged. An accumulation of dust was observed on toilet exhaust grill in Patient Room 515, Patient Room 532, Patient Room 529 and Patient Room 536. A section of the wallpaper was separated and peeling from the wall in Patient Room 536.
3) On the fourth floor, in Patient Room 416, a section of paint was bubbling up and a chair had damage seat upholstery. In Patient Room 417 a chair had damage seat upholstery. In Patient Room 415 the wall was damaged in the corner and 2 chairs had damaged seat upholstery. There was also one ceiling tile that had brown stains. An accumulation of dust was observed on the patient headwall light fixture and toilet exhaust grill in Patient Room 436. In Patient Room 430 the toilet exhaust grille had an accumulation of dust and wall damage under the window.
4) On the third floor, the edge of the door to Patient Room 318 was damaged and a portion of the wall paint was chipped. In Patient Room 317 the wallpaper was separating from the wall at one location.
5) On the first floor, two ceiling tiles with brown stains were observed in Patient Room 124. The wall under the window in Patient Room 111 was damaged and there was an accumulation of dust on the toilet exhaust grille.

B. The Director of Plant Operations was made aware of and verified each of the above at the time of observation.
