Based on observation and interview, the facility failed to ensure the two hour fire barrier separating it from a business occupancy was free of gaps. This has the potential to affect all patients and visitors to the facility.

Findings include:

On 11/02/16 at 11:12 AM a tour was taken of the building with Staff M, Q, and R.

On 11/02/16 at 11:45 AM observation above the drop down ceiling of the 90 degree angle outside of the laboratory and within lobby D200 revealed it did not extend all the way to the deck, leaving a half inch gap.

On 11/02/16 at 11:45 AM in an interview, Staff R confirmed the observation.

Based on observation and interview, the facility failed to ensure protective construction around each hazardous area was free of penetrating gaps. This has the potential to affect all patients and visitors to the facility.

Findings include:

On 11/02/16 at 11:12 AM a tour was taken of the facility was Staff M, Q, and R.

On 11/02/16 at 2:00 PM observation above the drop down ceiling of the soiled utility room located on the D2 unit revealed the I-beam in the upper left corner had protective construction built around it; however, the construction had open seams.

Based on observation and interview, the facility failed to ensure doors in and construction of two hour fire barriers were in accordance with NFPA 101, 2000 edition. This has the potential to affect all patients and visitors to the facility.

Findings include:

On 11/02/16 at 9:00 AM a tour was taken of the building with Staff M, Q, and R.

On 11/02/16 at 10:10 AM observation above the drop down ceiling of the two hour fire barrier running 180 degrees between the IT office and the corridor revealed unfilled corrugated spaces between the barrier and the corrugated deck.

On 11/02/16 at 10:10 AM in an interview, Staff Q and M confirmed the observation.

On 11/02/16 at 10:30 AM observation of the double doors in the two hour barrier leading to the gym, separating corridor 1115 and 1118 revealed the 20 minute rated doors were replaced by 90 minute fire doors. However, as installed, there was a gap of 3/8ths of an inch between the leaves.

On 11/02/16 at 10:30 AM in an interview, Staff Q confirmed the observation.

On 11/02/16 at 10:35 AM observation above the drop down ceiling of the two hour barrier between the LAN administrator office and the LAN workroom revealed, upon observation behind the silver colored insulation, that the barrier was constructed of one layer of drywall.
